Your name, complete mailing address, phone number and your email address in case we need to contact you.

The titles you are ordering (please be specific!)

Your credit card type, account number, and expiration date (MM/YY).

The name the card is listed under.

We'll be glad to calculate the total amount with postage (see the ordering information page). Remember, the seventh tape is free!

Please note, Visa & Mastercard orders require a 3% processing fee, American Express a 4.2% fee, we will figure this for you. Also, American Express orders must be shipped to your Amex billing address.
